HealthSnap and Sentara Health Launch Enterprise Remote Monitoring Pilot Across 389 Providers
Partnership aims to cut readmissions for 85,000+ chronic-disease patients in Virginia and North Carolina
NORFOLK, Va. – November 22, 2025 – HealthSnap, a Miami-based virtual-care management company, today announced a 90-day pilot with Sentara Health that will deploy remote patient monitoring (RPM) and chronic-care management (CCM) services to 389 primary-care providers and an initial cohort of 5,000 high-risk patients living with hypertension, diabetes and heart failure.
The pilot, which begins December 1, positions Sentara—an integrated, not-for-profit system with 12 hospitals and one million health-plan members—as the latest large U.S. health system to test whether daily physiologic data streamed from patients’ homes can avert costly admissions. If interim targets are met, the program will scale to the system’s full chronic-care population of roughly 85,000 eligible beneficiaries in 2026.
Remote monitoring is moving from grant-funded experiment to reimbursed standard of care. Medicare’s Physician Fee Schedule now pays clinicians an average of $172 per patient per month for RPM and CCM services when physiologic data—blood pressure, weight, glucose, oxygen saturation—are reviewed for at least 20 minutes. A 2025 Deloitte actuarial review projects the U.S. RPM market to reach $94 billion by 2028, driven by 21 million seniors expected to enroll in Medicare Advantage plans that carry RPM quality bonuses .

During the pilot, care teams will receive FDA-cleared cellular devices (blood-pressure cuffs, glucometers, weight scales) that transmit readings to HealthSnap’s cloud dashboard. An AI rules engine flags values outside custom thresholds and auto-generates reimbursable clinical tasks inside Sentara’s Epic electronic health record. Patients who miss readings receive automated SMS reminders and can chat with nurses through a HIPAA-compliant mobile app.
Early evidence suggests the model saves money. A 2025 study of 1,200 HealthSnap users published in the American Journal of Managed Care found a 38 percent reduction in 30-day readmissions and a $3,400 per-member per-year net savings among heart-failure patients. Similar results posted this month by the Commonwealth of Virginia showed every dollar spent on RPM returned $2.34 in avoided hospital days.
“Value-based contracts make prevention profitable,” noted Dr. Kara Hawkins, medical director of Sentara’s ambulatory RPM program. “If we prevent one admission, we’ve funded the entire year of monitoring for 20 patients.”
HealthSnap, founded in 2015, now serves 150 health systems across 33 states and has monitored more than 100,000 lives. The company closed a $25 million Series B round in March 2025 led by Endeavor Capital and says the Sentara pilot will serve as a template for additional health-system launches slated for Q1 2026.
Enrollment for the first 5,000 patients opened November 18 through Sentara’s primary-care offices in Hampton Roads, Richmond and western North Carolina. Patients must have a qualifying chronic condition, two or more hospital admissions in the past 12 months and a referral from their primary-care physician. Devices ship within 48 hours of consent, and nurses begin outreach within 24 hours of the first transmission.
